In an idealized classicist landscape, Saint Carlo Borromeo stands next to a huge marble sarcophagus depicting in bas-relief an adoration of the shepherds. The performance was based on a painting attributed to Rafael from c.1635. Borromeo points to the relief as he addresses two men, including a deacon. A crippled man sits next to him in front of the sarcophagus. On the left in the foreground, the Holy Family rides donkeys with the Christ child, accompanied by a young man. In the middle, a blind boy and a blind girl cross a stream of water. On the right of the second plan, a richly dressed man gives handouts to a group of beggars.
